Terms and Conditions

Client Agreement & Code of Care

 

  • The client will be treated with respect and care at all times. 

  • Disclosure of all information during therapy and consultations remains confidential. 

  • The hypnotherapist has a professional obligation to report to relevant authorities any suspicion of abuse or harm to a child. 

  • The hypnotherapist has a professional obligation to report to relevant authorities any concerns if they believe the client may be intending to cause harm to themselves, the therapist, or others. 

  • A query on suitability or conflict of therapy with other treatment practitioners may have to be sought occasionally, with client knowledge. 

  • If receiving medical treatment of any kind, it is recommended that proper diagnosis is sought where relevant, to assist the therapist and also to inform those professionals of your enquiries toward Hypnotherapy. 

  • A full copy of the AfSFH is readily available and can be provided electronically on request. 

Therapy Consent

 

  • The therapist has fully explained the procedures and treatment, together with any self-help on my part. 

  • I accept the fee payable and note the 24hrs notice of cancellation of appointment that is required. In instances where 24 hours' notice is not given, I agree to pay 100% of the session fee or lose the session if part of a pre-paid programme. 

  • I understand that the therapist needs to be able to see me at all times during the therapy session. I will remain on camera for the entire duration of the session, unless a break has been agreed with the therapist (for example for a trip to the toilet or to get a refreshment). 

  • I will ensure that I am in a comfortable, quiet environment, where I will not be disturbed for the duration of the session. 

  • The therapist reserves the right to refuse or postpone treatment if they feel threatened or disrespected. 

 

Parental / Guardian's Consent

I understand that my child   is embarking on a course of Solution Focused Hypnotherapy and I will encourage and support them in attending regular sessions in a timely manner and, where avoidable, to give 24 hours' notice if they are unable to attend. In instances where 24 hours' notice is not given, I agree to pay 100% of the session fee or lose the session if part of a pre-paid programme. 

  • I will encourage my child to participate in activities between sessions as set out by the therapist and I understand that, by doing so, my child will gain maximum benefit from each session. 

  • I will ensure that my child is in a comfortable, quiet environment, where they will not be disturbed for the duration of the session so their right to confidentiality is maintained. 

  • I understand that a block of 6 sessions must be completed within a 6-month period. After 6 months, even if I haven't had the 6 sessions I pre-paid for, I understand I will have to pay for any further sessions. 

  • I have had the process explained to me and understand what is entailed. 

  • I understand that the duration of therapy predicted by the therapist is an estimate and it may take longer than initially planned. I also understand that whilst the therapist has predicted the results based on previous experience, there are no guarantees of the outcome of the therapy. 

  • I recognise that the notes from my child's sessions will be held securely until their 25th birthday in compliance with the therapist's regulatory body, GDPR guidelines and for insurance purposes. 

  • I give my consent for the therapist to work alone with my child where appropriate and I understand that I can request to see a DBS (Disclosure and Barring Service) certificate at any point. 

  • I have been informed that once therapy has been completed, the therapist will remove my number from their phone and delete prior communication in a timely fashion. 

  • I have been given the chance to ask questions and accept these terms and conditions.
